Be a resident of New Jersey.
Meet income guidelines (185% of the Federal Poverty Income Guidelines).
Be pregnant, breastfeeding, postpartum (up to six months), or have an infant or child under five years old.
Have a medical or nutritional risk determined by a health professional.
