Office Information:

The Alliance Medical Center (AMC) offers WIC services in northern Sonoma County, with sites in Windsor, Healdsburg, and Cloverdale.

Call them at (707) 431-0831 to begin your application, find out what you need to bring in, and make an appointment to complete the application process.

At your appointment we will determine whether or not you and/or your children qualify for WIC.

You will receive an orientation to the WIC Program, including how to use the food checks, and you will meet individually with a nutrition counselor to talk about health and nutrition.

What to bring to apply for WIC:

•     Proof of identity for yourself, and for any child or infant that you are enrolling
•     Proof of income
•     Proof of your address
•     Your child or infant, if you are enrolling them
•     Medical referral form,  and WIC nutrition questionnaire

The Official WIC program is available to low to moderate income pregnant women, recently delivered women, breastfeeding women, infants, and children up to age 5 who are at nutrition risk. Fathers can also bring their children to apply for WIC. You may apply for WIC if you are working or unemployed. Check the Income Guidelines Table to see if you are eligible for WIC.
