You might qualify if you:
Live in the state of Oklahoma.Have a family income within program limits If you receive Medicaid, SNAP or TANF, you are automatically eligible for WIC benefits.Requirements:
Completed WIC Health Assessment for Child, WIC Health Assessment for Infant, WIC Health Assessment for Pregnant Woman, WIC Health Assessment for Postpartum Woman.Proof of Identification/Photo ID, Birth Certificate, Immunization Record, Proof of Income, Proof of Address, Medicaid Cards/SoonerCare cards.Call the Cleveland County Health Department Norman office at (405)321-4048 or the Moore office at (405)794-1591 to set up an appointment between 8:00 AM and 5:00 PM on Monday to Friday.




The Official WIC program is available to low to moderate income pregnant women, recently delivered women, breastfeeding women, infants, and children up to age 5 who are at nutrition risk. Fathers can also bring their children to apply for WIC. You may apply for WIC if you are working or unemployed. Check the Income Guidelines Table to see if you are eligible for WIC.
